I think the list reflects the era in which the moments in question reached the silver screen. The rape scene or the guy's head coming apart in Irreversible were repellent but in light of some of the stuff that passes the censors nowadays they weren't really that shocking. An audience in the early seventies watching a little girl swearing like a sailor and stabbing herself in her flange with a crucifix must have been pretty well fucking shocked.

In recent memory for me some of the most shocking moments in cinema have been Samuel Jackson's performances in Star Wars and Rocky 5. All of Rocky 5.
