What is this film! Argh?!?

Right, want to get this film I remember watching it a while back but i can't remember what it was called.

It was an american horror film where this doctor went round killing people, and as he killed them he would laugh in a really funny high pitched way. It was made around late 80's or early 90's.
I remember that a distinct few scenes were set in a funfair if that helps.

Come on, let me know what it is! I think the title is Dr G..........(something)
But i'm not really sure
