Mixed Media Disk

Anyone have any idea what causes this error ?
I have previously burnt a copy of Fifa 09 and all worked fine. Today I did another copy using the same image file. I got the Mixed Media Disk error. So I tried burning anothercopy of it and I keep getting the same error, all my other games load fine.
I'm using, as I always have.

When I put the disk in the PC it opens Media Player and says you need to put it in an XBOX360 as do all my disks
Any suggestions ?

You forgot the layer break. AKA you didn't make or use a /dvd file when you burnt it back.

Set the layer break to 1913760 as I did on the first copy that worked fine, I did everything the same as before, yet now getting this problem.
