Try getting the IOS16 WAD (IOS16-64-v257-prod.wad) and install that first.

Do you have enough spare blocks on your wii?
    What is the error number?

I had this problem on my Wii when i had the wrong CIOS installed - some of them block the wad loaderand other things from working properly. I followed the thread elsewhere on here and downgraded to 3.2 and it all works fine again.

If I remember correctly it was IOS37-64-v2070.wad that caused the problems and after I had downgraded I switched to IOS36-64-v1042.wad
