In future buy it direct from Apple outright.
It is what I do, no contracts so I am free to go with whoever I want and whenever I like. Plus in the long term it works out considerably cheaper.
Then you don't have to go through this process

£25 is pretty cheap and you should count yourself lucky, some unlocks cost over £80 if your network isn't willing. iPhones are unlocked on the Apple servers so it's not like the old days of buying a box, some cables and unlocking unlimited phones with them. You have to pay for each IMEI altered on the server.
O2 are one of the decent networks who unlock for free though

Three charged me £15 a couple weeks ago to unlock my 4S. Think EE and Voda charge £20
Remember with these unlocks though you have to plug them into iTunes.
Every time you restore the phone you need to repeat this.

Yes, if your carrier applies to get it unlocked then this is done by syncing to iTunes in order to do the unlock.
If you ever need to restore your phone then it has to do this again. You do not need to contact the provider again, you just need to sync it with an iTunes account.
I had to do this with a 3GS before, I do not think it is locked to your iTunes account it is just something iTunes looks at in a database and applies.

This is certainly how it was when I had a 3GS yes.
The Software updates were flakey back then and I remember twice having to restore after an update.
It could be because I was doing it from a back up prior to the unlock I am not sure. Maybe it has changed now but it was the case certainly with the 3GS that you had to sync to iTunes for the unlock to happen with the new SIM card in too.

No, when you restore any iPhone it requires activation before it can be used, this can be activated by connecting to any source of internet (iTunes, WiFi, compatible sim) so the phone can contact the Apple servers to gets it's status (unlocked, blocked etc). AFAIK an unlocked phone when restored would accept any sim to contact the servers whereas a locked phone would only accept the sim from the network lock.
