Help Sharepoint Online - help appreciated

Anyone know Javascript and Sharepoint Online?

    Scenario

Creating a new List item on a Sharepoint site - Before the Item is displayed and filled in, you first get some T&C's text and a small tick-box to show they have read the above text - once ticked then the rest of the list appears ready to be filled in.

Would this be best suited to a workflow or a custom action - if so, how would be best to go about this?

    Thanks in advance.

It's for a local Job Centre.

I have a Client list in Sharepoint Online - when I click on 'Create New Item' they want some text to be at the top of the page with some bog-standard Data Protection text with a small tickbox to say they have read and agreed to the above before proceeding - when the staff member ticks the box then the form will appear beneath ready to be filled in.

    Does that help at all?

    Cheers

Yeah, you shouldn't be using share point for this. Off the top of my head nothing amazing springs to mind but Mira would be easier to set up and maintain workflows.

Sorry that should have read Jira ive just noticed - you can set up workflows and it assigns it to another team/person once each stage is complete. You can have it send or even read in emails too
