Someone please explain to me the difference between Action plan and Roadmap, this in regards let's say and organizational transformation, or any other project. Is there a difference?

I think roadmap refers to the direction a business is heading in or the slated dates for the delivery of certain goals over a lengthy period.
An action plan would be a plan of action for a particular event, business strategy or singular business activity.
Thats my understanding anyway and could be fucking miles off.

Action plans are when you have identified a fault and you present a plan that people have to follow to prevent previous mistakes occurring again. If they do occur again then someone can get the blame for not following the action plan.
Roadmaps are basically cop outs,problem is identified but noone knows how to solve it so methods are suggested to improve the situation.

Roadmap refers an end goal and the strategy of getting there. Usually complex, hence the reference to mapping.
Action plan, a simple short term strategy either for solving an issue or achieving a short term goal.
