I have mine extracted to a external drive - open quickboot hit scan xex and point it to where the game folder is - that picks up the default.xex - then hit build then it asks if you want to upload to 360 say yes stick in your ip address and have xexmenu open or whatever to ftp - it then sticks the files on your 360 HD.

Games then show up in NXE and boot from external.

the problem with that is rto360 extracts the iso strait to the 360 so i cant send quick boot the games folder!

Edit: i have managed it now thanks to prezzy i had the wrong idea about quickboot and thought i needed it to scan the orig file but now i realised i can give it all the details it needs
