Usually software based from the manufacturer of the card...

    http://www.retire-asia.com/microsd.shtml

Failing that, use GParted to delete the entire card, create a new partition. Solved.

Obviously, you'll need to be able to plug your card into and adapter so a PC can read it.
Always format it to Fat32 on a phone.

Phones are usually MicroSD cards without the switch.

couldn't you just put it in an adapter and flick the switch on the adapter? if all else fails I'd go the gparted route, I dont use gparted as much as I used to but it will format the card regardless of what windows will tell you you can download it as a live distro and its only something like 55mb and in the past I've done the same when I had a confused 8gb microSD

http://www.filecluster.com/downloads...rmat-Tool.html i use this on my SD`s for my PI. agreed gparted very good you can download virtual box mount and boot gparted then mount SD card which enables you to format or resize.
