I know a guy who is mega into his security. Keeps all his passwords etc in some hidden unsent folder in an email address he uses only for this purpose.
So far he hasn't had any security issues and he's done this for over a decade.
Personally I use the same password and PIN for absolutely everything. But obviously I've made it really obscure, I'm not an idiot.

most of my passwords are variations of the same password. never had anything hacked, apart from a googlemail account which i never use anyway.
phone pins tend to be the same as my bank card pin, which get changed regularly. Each of my important apps and folders on my iphone are individually locked thanks to a tweak called Lockdown Pro. I just tend to remember the codes for each.
Never write anything down
The one I've heard of that's meant to be good generates a random 16 (at a rough guess) character password that is letters, numbers, symbols etc. these are linked to the website and username for the website, encrypted and you only need one master password. Then if you ever have one of those ebays been hacked type things you don't have to change all your passwords everywhere and they're strong ones too.
Lastpass premium, it has a browser extension and iphone app. Use a very strong password to get into it then have access to all of your other details.
I personally think using apps to keep password's has to be a security breach all of it's own, there's no way you can guarantee those password's are safe in an app.
Admittedly it's getting tough now trying to remember so many password's and pins that if I forget I dont worry and just use the 'forgotten password' link.

All my passwords and pin numbers, plus my wife and my son's have always been in my head. However, due to security breeches and databases becoming compromised, I'm considering moving all my details into something like Keepass and just having completely random passwords verses something meaningful - http://keepass.info/
Ive started using KeePass with the database stored in Google Drive which syncs with a couple of PCs and a very strong password on it. I used to have a system that made it easy to remember passwords for each site but one site had its DB leaked so when I had to change it the system was fooked hence moving to KeePass.

Passwords I do as per Callme they are a variation on the same but I have 3 passwrds to vary if shitty site then use this one emails use another one and banks use even stronger one.
PINS I have a fake person in my phone book and put the PINS in a certain placement in the phone number
For example if I had the PINs (in example is 1234) at the end of the fake no's I would have
Mobile 0780104 1234
Work 01564 52 1234
etc
I trust this method more than a password app as its only my head that knows the algorithm/fake entry.

Being a network manager I have to remember far too many numbers/passwords never thought about it before now but there are more than I can care to count.
